Overview: As a LPN, you will work in our newly renovated state of the art facility that specializes in focusing on patient centered care in our traditional long term care setting or within our memory loss unit. At our 5 Star and Gold Seal status facility you'll be trusted with the care of our valued residents.

How to make an impact:

Provide direct skilled bedside nursing care and work with patients and families to provide comfort and preserve dignity in the face of terminal illness in the home setting.

Evaluate patient /family needs in the preparation and executions of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors patient’s reactions.

Participates in Nurse LPN on- call rotation/schedule in order to meet the needs of patients and families.

Administer?skilled nursing care with appropriate documentation, under the supervision of a registered nurse and the physician directed plan of treatment.

Provide nursing services in accordance with physicians' orders, under the?direction of an RN,?in line with state licensure status, and in compliance with agency policies and procedures, applicable health care standards, and governmental laws and regulations.?

Assess individual status and care/instruction needed by the client.

What you’ll bring to the table:

• Current Florida LPN License

• Current CPR/BLS Card

• IV certification required

• Minimum of 1 year of experience in a Long Term Care, Skilled Nursing Facility, or Hospital

• Ability to communicate clearly through written, electronic, and verbal means
