PositionSummary:

• To determine the medical necessity of alladmissions and the need for continued stay. To identify the provision of inappropriate or untimely services.  To assess discharge planning needs forpatients of all ages and coordinate the delivery of services to meet thoseneeds.  To facilitate timely andappropriate discharges from the hospital. To identify opportunities for improvements in patient care.  To provide documentation of all functions asrequired by regulatory agencies.
• Case Management Social Worker plans, organizes,directs, defines and coordinates clinical services necessary to meet the legal,organizational, medical staff and regulatory agencies guidelines. Provide casemanagement, education, group and individual therapy, and discharge planning.Performs community and educational activities.  

Qualifications

• Bachelor’s degree in Social Work; Master’sDegree preferred
• Experience in an acute hospital setting highlypreferred, plus discharge planning experience
• 1+ years of professional experience withinbehavioral health settings
