We are seeking a Social Worker for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. What you willdo in this role:
Youwill facilitate the interdisciplinary plan of care with a focus on evaluatingthe appropriateness of clinical care, medical necessity, admission status,level of care, and resource management.
Youwill coordinate activities that promote quality outcomes and patient throughputwhile supporting a balance of optimal care and appropriate resourceutilization.
Youwill identify potential barriers to patient throughput and quality outcomes
Youwill evaluate the psychosocial needs of patients and family support systems andcoordinate appropriate discharge plans for identified patient populations.
Youwill act as a liaison between the facility and resources external to theorganization.
Youwill help with the facilitation and implementation of timely discharge plansand facilitate follow-up to anticipated post-acute interventions identified inthe plan of care.
Youwill provide crisis intervention and support to patients/family support systems.
Qualifications
Licensure as a LMSW is required. LCSW is highly preferred
Master’s degree in Social Work Required
3-5 years experience in hospital patient care preferred
