Position Description:

Organizes andfacilitates activities associated with Performance Improvement for thehospital’s Sepsis & Stroke programs. Interacts with members of the Medical Staff, Nursing Department andother hospital department leaders and staff to ensure organization-wideunderstanding of program requirements. Employs techniques and tools associated with continuous PerformanceImprovement to assist teams in achieving goals for improvement. Worksextensively with physicians, pharmacists, nurses, and other hospital staff inachieving support for the practice of evidence-based medicine related toclinical care of sepsis and stroke patients. 

 

The Sepsis & StrokeCoordinator position is responsible for the data abstraction, aggregation andanalysis of population data using Comet systems for input of data, analysis,comparison, and trending.  TheCoordinator provides educational support organization-wide as necessaryregarding implementation of evidence-based initiatives, interpretation of theassociated practice requirements, and data analysis.  The Coordinator performs clinical concurrentreview, conducts team meetings, and coordinates internal reporting.  The Coordinator maintains an in-depthknowledge of hospital systems, patient clinical data, Meditech capabilities andreports, COMET capabilities and reports and external data bases used for publicreporting and comparison.  TheCoordinator ensures the integrity of the data, and adheres to abstraction rulesof CMS, TJC, HCA Outcomes Measurement Department, and Ethics and Compliancepolicies.  The Coordinator developssystems for evaluating outcomes for clinical departments and physiciancompliance.  The Coordinator facilitatesSepsis and Stroke-related Teams/task forces/Committees and initiates andcoordinates an in-depth analysis on near or actual misses in compliance incollaboration with risk and quality staff, and develops strategies to achievethe desired results of measure sets.

Qualifications

  • Associate Degree in Nursing required; Bachelor's Degree in Nursingpreferred
  • FL RN Licensure, or
  • Recognized CompactLicensure (subject to Florida State Licensing Requirements, including/notlimited to ongoing eligibility and duration provisions)
  • BLS Certification
  • ACLS Certification
  • 1 – 3+ years of related work experience, preferably in an acute caresetting
  • Meditech,Word, Excel, Powerpoint and/or other presentation programs
  • Attention to detail a must with strong ability to concentrate on detail
  • Word Processing, copiers, fax, COMET application, ability to interpretguidelines accurately and teach to others
