Job Description:

The NurseNavigator’s primary function is to ensure the patient remains compliant to the treatmentplan. Navigators achieve this goal by building relationships with patients andphysicians, coordinating the plan of care, assisting with appointments,transportation needs, education, resource provision and/or representationwithin the multidisciplinary care environment. The Navigator also assumesresponsibility and accountability for the management of resources to achieveefficient, high-quality outcomes for cancer patients, including support withinterdisciplinary and cross-facility coverage and collaboration.

 

The Navigatorwill serve as a liaison between the patient and family, all physicians involvedin that patient’s care, internal and external healthcare providers, supportnetwork members, and the wider healthcare community. This role will requirecollaboration with local physician liaisons and leadership to conduct internaland external outreach and marketing.


Qualifications

  • Associates of Science in Nursing - Required
  • Bachelor’s degree in Nursing - Preferred
  • 3+ years of experience in Acute Care Setting - Required
