Job Descripition:
The Care Assure Nurse Navigator is responsible for theprograms and resources that support the navigation of patients through complexhealthcare environments for the optimization of care and outcomes. S/hefacilitates patient follow-up through coaching, information transfer, andcommunication in order to improve continuity of care, enhance the medicalmanagement of complex disease, and streamline the intervention process. Theincumbent guides patients through the health care system by assisting withaccess issues, developing relationships with service providers, and trackinginterventions and outcomes. Serves as a point of contact for referringphysicians, patients, and caregivers to provide resources and assistance withaccessing clinical and supportive care services.
Job Duties:
- Schedule follow-up appointments, andprovides information with reminders to patients and physician offices
- Engage providers and patients using aprogram that improves outcomes and efficiencies in health care
- Facilitate ongoing performanceimprovement activities related to the target population and involving alllevels of staff ·
- Coordinate information among referring,primary care and consulting physicians and ancillary departments
- Evaluate and provides input regarding theutilization of resources to meet the needs of the target population

Qualifications
- Current RN License in the State of Florida
- RecognizedCompact Licensure (subject to Florida State Licensing Requirements,including/not limited to ongoing eligibility and duration provisions)
- Graduate from a school of nursing (ASN) required. BSN preferred.
- Minimum of 3 years of acute hospital experience required
- Current American Heart Association Basic Life Support (BLS) Certification.
- Current (ACLS) Advanced Cardiac Life Support Certification.
- Fluent in Spanish preferred.
- MS Office, hospital based computing systems (i.e. Meditech), criticalthinking, excellent interpersonal communications skills, PC Skills required.
