PRN Case Manager (RN) - Case Management Rehab (IRF)

Job Summary: 

Ensuresappropriate utilization of resources, facilities, and services by the review ofadmission, continued stay, and professional services rendered.  Identifies patient discharge needs and worksin collaboration with nurses and physicians in managing the patients care needsalong the continuum.  Establishes andmaintains a good rapport and professional relationship with fellow employees,physicians, patients, and other departments. Follows policies and procedures ofWest Florida Hospital. Participates in the Quality Improvement process andadheres to WFH standards of behavior.

Essential Functions (including, but notlimited to): 

Managesstress appropriately and can make decisions under pressure, handle multiplepriorities, work in areas that are confined or crowded. Light to medium workwith periods of standing, lifting (50 lbs), stooping, bending, walking,talking, answering telephones, using computer, push/pulling with force andreaching above head and shoulders.  Mustbe able to distinguish colors, hear alarms and blood pressures.  Good manual dexterity and clarity of vision.Attendance is essential to this role.

Qualifications:

• Associate’sDegree (or Diploma) in Nursing is required,BSN preferred
• FLRN license and/or approved multistate license required
• A minimum ofone (1) to three (3) years of nursing experience required
• Case managementexperience preferred
