POSITION SUMMARY:  The RN Case Management Care Coordinator providesongoing support and expertise through comprehensive assessment, planning,implementation, and overall evaluation of individual patient needs.  Enhances the quality of patient managementand satisfaction to promote continuity of care and cost effectiveness throughthe integration of case management, utilization review management and dischargeplanning.

QUALIFICATIONS:

• RNdiploma program or completion of related health care program required
• Minimumof three years recent acute clinical practice or related health care experiencerequired
• FloridaRN license required; or appropriatecompact license; or ARNP

SKILLS, KNOWLEDGE &ABILITIES:

Criticalthinking, service excellence and good interpersonal communication skills,ability to read/comprehend written instructions, ability to follow verbalinstructions, PC skills, Demonstrates analytical and critical thinkingabilities with pro-active decision-making and negotiation skills, strongknowledge of governmental and private organization and community resources.
