POSITION SUMMARY:

Provides hospital case management/utilizationreview and discharge planning to assure that the patient progresses through thecontinuum of care and is discharged to the least restrictive environment.   Coordinate the integration of the socialservice function into patient care. Coordinate the hospital activities concerned with case management anddischarge planning.   Adhere todepartmental goals, objectives, standards of performance, and policies andprocedures.  Ensure compliance withquality patient care and regulatory compliance. 

QUALIFICATIONS:

• Nursing Degree required
• Current Registered Nurse (RN) license in State of Florida required; or appropriate compact licensure
• One (1) year experience in case management and/or supervisory /management positioninvolving case management/utilization review/responsibility in an acute carehospital and/or managed care setting required.
