We are seeking a Case Manager Registered Nurse for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care.  What you will do in this role:



  • You will beresponsible for coordinating the plan of care for the patient stay, managingthe length of stay, ensuring appropriate resource management and developing asafe appropriate discharge plan in collaboration with the multidisciplinaryteam.

  • You willfacilitate the progression and transition of care using established criteriaand in conjunction with the multidisciplinary team.

  • You will Identifypatients who are at risk for adverse outcomes during the transition from onelevel of care/setting to another

     

  • You will perform acomprehensive assessment of psychosocial, medical and discharge needs ofpatients/family along with an assessment of resources appropriate and availableto the patient/family


 


  • You will reassessthe patient’s clinical condition as indicated. Considers patient’s readmissionstatus or risk of readmission and develops strategies to mitigate includingeducation on appropriately accessing healthcare resources, preventativeeducation, and community based resources


 


  • In partnershipwith Social Services, you will be responsible for ensuring the post-acutemedical needs and level of care are appropriate


 


  • You will evaluateprogression of care using evidence-based tools and approved criteria(InterQual) throughout the episode of care; escalates progression andtransition of care issues through the established chain of command


 


  • You will makeappropriate referrals to third party payer, disease and case managementprograms for recurring patients and patients with chronic disease states


 


  • You will act as aliaison through effective and professional communications between and withphysicians, patient / family, hospital staff, and outside agencies


 


  • You will serve as an advocate for patient's rights,needs, and values; ensuring that patients’ ethnic, cultural, or religiousvalues, beliefs, preferences and needs are considered and aligned


   

 

 

 

 

 

 

 

 


Qualifications

 

  • Must be licensed as a Registered Nurse in accordance with state regulations
  • Graduation from an accredited school of registered nursing is required. BSN highly preferred
  • 1-3 years’ clinical, hospital nursing experience required.  2 years of experience in case management or critical care preferred
  • Certification in case management or utilization review preferred
  • InterQual experience preferred
