Case Manager - Healthcare

The Position: Our Client is looking for a Case Manager. The Case Manager will manage the daily administrative duties for the program to include referrals, patient follow up, connecting patients to specialty care, eligibility, program billing, and working with other staff and systems to ensure that patients are connected to resources including psycho-social services. The Client is offering the following salary and benefits:

• $35,000 to $40,000 Depending on Experience

Requirements:

• Case Management Experience Required
• Experience with Ryan White or similar programs preferred
• Experience with data input and managing databases
• Experience with Excel

Responsibilities:

• Maintain and Monitor Patient Database and Complete Any Paperwork/Applications Related to the Management of the Patient Care Experience
• Work with a Multidisciplinary Team to Ensure that Patients’ Medical, Dental and Mental Health Needs are Addressed.
